- A copy of Mahidol University Student ID card (Certified true copy)
3.2 Mahidol University Staff and family
- A copy of Mahidol University Staff ID card (Certified true copy)
- For spouses, an additional copy of the marriage certificate and a copy of ID card / a copy of passport (Certified true copy) are required.
- For children, an additional copy of the household registration / birth certification (Certified true copy) is required.
- In case of staffs children applying for membership of SS Sports Center, applicants must be at least 15 years old.
3.3 Mahidol University Alumni
- A copy of ID card / a copy of passport (Certified true copy).
- A copy of diploma (Certified true copy).
3.4 Civil servant
- A copy of civil servant ID card (Certified true copy).

- A copy of ID card / a copy of passport (Certified true copy).
